Huge Facial Nerve Schwannoma in Middle Cranial Fossa

Abstract
A 59-year-old female with a 3 month history of dysarthria was presented with confusion and depression without facial palsy. Magnetic resonance imaging (MRI) revealed a huge mass in the left middle cranial fossa. We performed a left fron-to-temporal craniotomy. Microscopic study showed that the tumor was a schwannoma. The patient suffered from facial palsy and cerebrospinal fluid (CSF) rhinorrhea during the postoperative period. Postoperative temporal bone computed tomography (CT) scans and MRI revealed a destructive process in the superior portion of the left petrous portion of the temporal bone and a tumor arising from the geniculate ganglion. A extradural middle fossa approach was employed and the mass was removed. The patient¡¯s CSF rhinorrhea was resolved, but facial palsy of House-Brackmann grade 3 remained after surgery. Neurosurgeons should remain aware of the possibility of consider facial nerve schwannomas when treating tumors extending to the middle cranial fossa even in the absence of facial palsy.
